1. I am participating in Yoga/Pilates classes, health programs, workshops and/or other wellness, body work, therapy, exercise and healing arts activities offered by Selph Health Studios and/or its owners, licensees, managers, teachers, instructors, workshop presenters, employees, independent contractors, subsidiaries and associated entities. I intend to participate in the activities at a particular studio operated by Selph Health
Studios.
2. I will follow all instructions given to me by Selph Health Studios and its teachers as to when, where and how to perform and not to perform the yoga/pilates/activities and
associated exercises prescribed by Selph Health Studios teachers and practitioners.
3. I recognise that I must be in adequate physical and mental health to participate in the activities. I understand that the activities may require intense physical exertion and I
accept that I am physically fit and able to participate and I have no medical conditions that would prevent my full participation in the activities. I understand that it is my
responsibility to consult with a doctor before my participation in the activities. If done so, I have taken the doctors advice. I understand that Selph Health Studios reserves the right to refuse my participation in any activity on medical, fitness or any other grounds.
4. I assure and will at all times hereafter assure Selph Health Studios, its owners, teachers, representatives or employees of Selph Health Studios from and against all actions, suits, causes of action, proceedings, claims, costs and expenses whatsoever which may be taken or made against Selph, its owners or anyone representatives of Selph Health Studios, incurred or become payable by Selph Health Studios, it’s owners, teachers or employees of Selph in connection with or arising out of any such injury,
illness or mishap to me.
5. I acknowledge that I have carefully read this document and fully understand its content. I voluntarily and knowingly agree to the terms and conditions stated herein. I am
aware that by signing this document, I am giving up substantial rights, including my right to sue, and certain legal rights my heirs, next of kin, executors, administrators and
assigns may have against Selph Health Studios.
